How Clomiphene Triggers the Hormonal Response

Clomiphene interacts with estrogen-receptor-containing tissues in the hypothalamus and pituitary. This alters estrogen feedback and can increase release of follicle-stimulating hormone and luteinizing hormone, supporting follicular development, a preovulatory hormone surge and release of an egg.

The response depends on the underlying cause of anovulation and the ability of the ovaries to respond. A positive ovulation test or follicle development confirms a hormonal response, but neither result guarantees pregnancy.

Who Needs Evaluation Before Treatment

Evaluation should exclude pregnancy and identify causes of ovulatory dysfunction that require different treatment. Clinicians may assess menstrual history, thyroid and prolactin disorders, ovarian reserve, pelvic anatomy, semen factors and tubal patency according to the patient’s history and treatment plan.

Clomiphene remains an option for some patients, but current evidence-based guidance generally prefers letrozole for ovulation induction in women with anovulatory PCOS and no other infertility factors. Timing the Tablets and Ovulation Window

Current labeling describes an initial course of one Clomid 50 mg tablet daily for five days in a carefully selected patient. Treatment commonly begins on or about the fifth day of the cycle, but the prescriber sets the exact starting day after pregnancy and contraindications have been excluded.

Ovulation most often occurs about five to ten days after a course, although timing varies. The prescriber may recommend urine luteinizing-hormone testing, basal body temperature tracking, ultrasound, progesterone testing or another method to identify whether and when ovulation occurred.

Confirming Ovulation and Deciding on Another Cycle

If ovulation does not occur after the initial course, the clinician may alter the next course within labeled limits. A prescribed total of 100 mg per day is commonly produced with two 50 mg tablets; it should not be presented as proof that a separate 100 mg tablet product is available.

Pelvic evaluation is recommended before treatment and before subsequent courses. Visual Symptoms Require Prompt Action

Blurred vision, flashes, spots, after-images or double vision may occur, and risk increases with greater total exposure. Although many cases resolve, prolonged and possibly irreversible visual disturbances have been reported.

Stop clomiphene and contact the prescriber immediately for any visual symptom, and obtain a prompt complete eye examination; seek urgent medical care for severe pelvic or abdominal pain, rapid swelling or weight gain, shortness of breath, fainting, or signs of a serious allergic reaction.

Do not drive or operate machinery when vision is affected, particularly in changing or low-light conditions. A new course should not be started until the symptom has been evaluated and the treating clinician has provided direction.

Ovarian Enlargement, OHSS and Multiple Pregnancy

Ovarian enlargement and cyst formation can occur, and ovarian hyperstimulation syndrome may progress rapidly over hours to days. Warning symptoms can include increasing pelvic or abdominal pain, distension, nausea, vomiting, diarrhea, rapid weight gain, reduced urination or shortness of breath.

Clomiphene also increases the chance of twins or a higher-order multiple pregnancy. Ultrasound monitoring may be appropriate when the clinician needs to assess follicle number, ovarian response or the risk of continuing a cycle.

Contraindications and Health Conditions to Review

Before treatment, the prescriber and pharmacist need a complete medication, supplement and medical history. Important exclusions and review points include:

• Pregnancy or a possible existing pregnancy.
• Liver disease or a history of liver dysfunction.
• Abnormal uterine bleeding that has not been diagnosed.
• Ovarian cyst or enlargement not caused by polycystic ovary syndrome.
• Uncontrolled thyroid or adrenal dysfunction, or an organic intracranial lesion such as a pituitary tumor.
